When is Lean too Lean?

“Lean” manufacturing or management is a popular program to improve the quality and productivity of laboratory operations, including cytology operations.  According to Wikipedia, “Lean,” is a production practice that considers the expenditure of resources for any goal other than the creation of value for the end customer to be wasteful, and thus a target for elimination…  Essentially, lean is centered on preserving value with less work. Lean manufacturing is a management philosophy derived mostly from the Toyota Production System (TPS).” This approach has been used to improve the performance of laboratories around the world.

Ultrasound- Guided Fine Needle Aspiration: Evaluating the Lung Mediastinum 






Endobronchial ultrasonography with transbronchial needle aspiration biopsy (EBUS-TBNA) is a minimally invasive technique, increasingly being used to sample
paratracheal, hilar or mediastinal lymph nodes or mass lesions for staging or diagnosis. EBUS-TBNA consists of fine
needle aspiration performed with the assistance of a bronchoscope with an integrated ultrasound probe, often followed by rapid on-site cytologic evaluation

FNA of an Inguinal Mass

A 44 year old man presented to the FNA clinic with a 2 cm right inguinal mass.  Two years earlier, the patient underwent resection of a 3 cm subcutaneous mass from the lateral aspect of his right foot.  A fine needle aspiration of the inguinal mass was performed by the pathologist.
