       What is the standard method of accessioning bronchoscopy cases? For instance, a patient undergoes a bronchoscopy and gets a bronchial washing, brushing and BAL. Should this case be accessioned as one case or multiple cases? Should you add -59 modifiers to the charges? I have been told that Medicare is honing in on these cases for overbilling.
