The ASC will cross-reference your current employee list with the ASC Membership files. Employees who are not current members of the ASC will be required to complete and ASC Membership Application and be approved by the ASC. Each year the ASC will provide you a list of your current ASC members for your review.

I certify that the information contained in this application is true and complete to the best of my knowledge and belief. The above laboratory will pay the membership fees for the ASC members listed above.
